Parent Volunteer Form
Yes! I would like to help assist in the following areas. You will be contacted to confirm before each activity. Please return this form to Mrs. Hall by September 14th, 2007 (or at your earliest convenience). Thank you!
______ Uniform – assist with fittings (prior to ordering, generally in mid September) and at concerts and festivals (check band calendar for dates of concerts and festivals: 2-3 people per event).
_______ Fund Raiser – help with form and money collection, accounting and distribution of items (Mid October – Mid November: 2-3 people)
_______ Chaperone (check below) – 1 chaperone per 10 students
(Checking does not guarantee you as a chaperone – nothing is finalized until you are contacted by Ms. Blye, prior to the event)

_____District Band Festival (February 29th – March 1st)
_____Spring Trip (May 23rd) – Hershey Park
